Municipal Court Forms

Appeal Packet (PDF): If you are not satisfied with the outcome of your court case you may file an appeal with the Essex County Superior Court. This packet contains a guide on how to file an appeal. Please print out the packet, complete it and bring it to Essex County Superior court with the correct fees.
Citizen Complaint Form (PDF): If you would like to file a complaint, read the instructions carefully on the Filing a Complaint (PDF) packet then complete the citizen complaint form and bring it with you to the violations bureau window so that either the Court Administrator, or Deputies may check for Probable Cause
Public Defender Form EN (PDF)/Public Defender Form ES (PDF): If you feel that your traffic/summons may require a public defender, please print, complete and bring this form with you the day of your court session.
Records Request Form (PDF): These forms are used for requesting a certified copy of any court document. You may print out the form, complete it and bring it to the violations bureau window, or send it by mail.
